After our initial consult you will be required to put down a deposit in order to secure a date and time.
Do not fill out a form if you are under the age of 18.
All appointments require a non-refundable deposit.

If you need to reschedule, you must provide at least a 48 hour notice to avoid losing your deposit.
Please only submit a form if you are prepared to make a deposit.
If you are more than 15 minutes late, you will need to reschedule your appointment and risk losing your deposit.
You may reschedule 2 times per appointment before your deposit is confiscated and you will need to place a new deposit in order to rebook.
